Choose the BCI product the recipient actually requires

Utah BCI maintains statewide submitted criminal arrest and outcome information. Its own-record service lets an individual review their Utah history; authorized employment, licensing and volunteer checks use different channels. A Utah personal report is not a national clearance, a court case file or a copy of an arresting agency’s report. Ask the recipient which product and delivery route it accepts before ordering.

The personal-record form says BCI does not maintain juvenile offender records and directs those requests to Juvenile Court. If you need proof of the outcome in a particular adult prosecution, use the filing court’s judgment or dismissal in addition to any BCI summary. A charge without a documented disposition should not be described as a conviction.

Visit the Taylorsville office for your own Utah history

The BCI office is at 4315 South 2700 West, Suite 1300, Taylorsville, UT 84129. Published hours are weekdays, 8 a.m.–5 p.m., excluding state holidays. The own-record fee is $20. Bring valid government-issued photo identification; a Utah Driving Privilege Card is not accepted. The July 2025 application says a fingerprint appointment is not required for this criminal-history report service.

Call 801-965-4445 if your identification or circumstances need clarification. BCI also publishes a Right of Access Agencies list; those agencies can provide a person’s own Utah history, but their policies, fees and hours must be checked individually. Do not assume every police counter offers the BCI service or can provide someone else’s record.

Prepare the mailed application without missing the fingerprint step

Use the Application for Criminal History Record linked from BCI’s forms page. Complete the identifying fields, previous names, addresses and signature. Take it with valid photo ID to a law-enforcement agency: the official records identification details and prints the four right-hand fingers in the form’s space. A ten-print card may be submitted instead. Include a photocopy of your ID.

Mail the completed application and $20 fee to the Taylorsville address above. Checks and money orders are in U.S. funds payable to Utah Bureau of Criminal Identification; the form also provides a credit-card payment section for Visa, Mastercard, Discover or American Express. Cash is accepted only in person. Do not send cash through the mail.

Set the delivery address before submission

The personal-history report is mailed to the address on the application, not sent by fax or email. To send it directly to another person or organization, include the Third Party Release form with the application. The release requires the named recipient, a complete mailing address and the applicant’s signature; an agency recipient should include the specific individual authorized to receive it.

The release’s email field is marked for expungement applications only, so filling it in does not turn this history request into electronic delivery. BCI’s reviewed personal-history instructions do not publish a guaranteed processing interval. Allow for delivery and ask BCI about a deadline before relying on receipt by a particular day. Ask the recipient separately whether it needs a court-certified disposition.

Use the new employment and volunteer fee schedule

BCI qualifies organizations for particular state and fingerprint-based federal checks; authority depends on the entity and purpose. Applicants must be notified and have an opportunity to review or challenge results. Qualifying entities receiving FBI information also provide the required privacy materials and signed-waiver process. Start with the organization’s instructions rather than sending a personal application under the wrong program.

Effective October 1, 2026, BCI lists WIN/FBI fingerprint submissions at $33 for volunteers and $35 for employees, licensees and contractors. RapBack adds a one-time $10 fee. These program prices are separate from the $20 Utah own-record request. Confirm which submission and any collection charge applies before paying.

There is a material difference in agency guidance: BCI’s “Other Employers” text suggests employees obtain a personal FBI summary, but the FBI’s own current page says that summary is not for employment or licensing. Do not assume the personal FBI product is an accepted substitute; resolve the required authorized route with the requesting organization and BCI before submitting.

Challenge inaccurate or incomplete BCI information

The current Record Challenge application costs $20. Mark the relevant challenge category, sign it, complete the required fingerprint/identification step and attach your record with the disputed information highlighted. Include supporting documents, such as the correct court disposition, so BCI can evaluate the actual discrepancy. The older 2023 form’s $15 figure is superseded by the July 2025 form and current fee notice.

A challenge asks BCI to correct completeness or accuracy; it is not an expungement application. If the challenged result is denied, the current form identifies a district-court appeal route. For that legal step, obtain advice about the applicable procedure rather than expecting a customer-service call to change the underlying judgment.
